Our supported living services are designed for adults aged 18 and above, including those transitioning into adulthood. We support people who need help with daily living, emotional wellbeing, life skills and community access. 3. How are care and support plans created? Each care or support plan is shaped around the person’s needs, preferences, strengths and goals. We work with the individual, their family and relevant professionals where appropriate. 4. Do you support people in their own homes? Yes, our domiciliary care service supports people to remain safe, comfortable and independent at home. Support can include personal care, daily tasks, companionship and help with routines. 5. How do I arrange a care assessment?
